Forms to be issued against original Mark Sheet of Higher Secondary (10+2)

Payment of Downloaded Application Form will be made at the time of submission of Application Form at the college counter. (Rs. 100/- each)

Cost of each Form with Prospectus Rs. 100/- each

Documents to be produced at the time of admission.

Original admit card of Madhyamik or any other age proof certificate

Original Mark Sheet of Class XII level.

Original S.C./S.T. Certificate (For S.C or S.T. candidate)

Original Migration certificate, wherever applicable

Documents to be attached with the form :

All Photo copies duly attested as mentioned in Item No.4

One recent stamp size photograph of the candidate to be pasted on the form.

All forms should be properly filled up, otherwise it will be cancelled.
